Each year, we open the door for clients, families, and our community members to share their thoughts with us. Your voice is essential in helping us shape our services to meet your needs. Whether you want to highlight what’s working well or share big ideas for the future, now is your chance to be heard!
We’re offering two ways to make giving feedback easy and accessible:
FULL SURVEY: Available on Survey Monkey, this survey takes about 8 minutes to complete. It includes space for detailed comments and feedback, so you can really let us know how we're doing.
EASY-READ SURVEY: For clients who might need some assistance, our easy-read version on Kahoots has pictures, simple questions, and a fun interactive format. Plus, you get to choose a nickname—go wild!
